MSW Social Worker

Therapy Exclusive is seeking contract Licensed Medical Social Worker (as needed basis, part time , per visit basis ) to do home health visits in DFW and surrounding areas for geriatric population for Home Health care agencies. The Social Worker, must be licensed in state of Texas as MSW or LBSW License with master or bachelor degree in social work . Primary function is to provide coordinated care in the home to clients of all age groups and perform psycho\-social assessments, analysis, counseling, and referrals to meet the needs of the patient/ family for home health care agencies . \- Flexible schedule \- you can choose your area and number of visits you would like to do in a week. \- Social worker license in state of TX is required \- Home health or Hospice experience is preferred \-Great compensation per visit Job Types: Part\-time, Contract Pay: $45\.00 \- $55\.00 per hour Benefits: * 401(k) matching Medical Specialty: * Home Health Application Question(s): * Do you have Medical Social Worker License in state of Texas * List the Area you can cover for home health visits * How many visits you can do per week Work Location: On the road
