In this role, you will provide clinical case management, care coordination, crisis\-response support, and psychological\-readiness services for Soldiers experiencing behavioral\-health concerns that may affect their well\-being, military readiness, retention, or deployability. You will work closely with Soldiers, commanders, medical professionals, military treatment facilities, civilian providers, and community organizations to coordinate appropriate care and promote continuity throughout the treatment and recovery process.
