We're seeking a **Personal Care Assistant (PCA)** to join a dedicated interdisciplinary team providing exceptional care to older adults in a welcoming Day Center environment. In this rewarding role, you'll provide hands\-on personal care, assist with daily activities, support participant engagement, and help create a safe, comfortable, and enriching experience for our seniors. **What You'll Do** ------------------ * Assist participants with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility. * Measure and document vital signs. * Safely transfer participants and assist with transportation as needed. * Escort participants to medical appointments when assigned. * Support meal service and assist participants with feeding when appropriate. * Observe participants throughout the day and promptly report any changes in condition. * Assist with recreational and engagement activities. * Support the Home Care and Nutrition teams as needed. * Maintain participant care supplies and keep participant areas clean and organized. * Complete accurate documentation using an Electronic Medical Record (EMR) system. * Follow all safety, confidentiality, and infection control policies. **What We're Looking For** -------------------------- * Minimum **1 year of experience** providing personal care to a frail or elderly population. * High School Diploma or GED. * Current **California Home Health Aide (HHA)** or **Certified Nursing Assistant (CNA)** certification. * Current **California Food Handler's Certificate**. * Experience using Electronic Medical Record (EMR) systems. * Excellent communication, organizational, an
