Your energy, empathy, and attention to detail will help clients thrive in their familiar surroundings. **Duties** * Provide personal care assistance including bathing, grooming, dressing, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques. * Assist with activities of daily living (ADLs) to promote independence and comfort for clients with various needs including dementia, Alzheimer’s, autism, developmental disabilities, and neurodevelopmental disorders. * Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals while adhering to HIPAA regulations. * Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation to support overall health. * Support medication administration under supervision or as directed by healthcare plans; ensure proper storage and documentation of medications. * Conduct light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote hygiene and safety within the home. * Provide transportation assistance for medical appointments or errands, ensuring client safety during travel and transfers. * Document care activities accurately in care plans and communicate effectively with clients’ families or healthcare teams to ensure consistent support. **Requirements** * Proven experience working with individuals with disabilities, developmental or intellectual disabilitie
