Caregiver

Under the direction and supervision of the Home Care Supervisor, the PACE Home Caregiver provides personal care, light housekeeping, and/or supervision to North East Medical Services PACE participants in the participant’s home. **ESSENTIAL JOB FUNCTIONS:*** Provides assistance in the participant’s home according to the participant’s individualized care plan, and in coordination with the PACE Interdisciplinary Team (IDT), the participant, and the participant’s family. This may include assistance with: + Personal care including tasks such as peri\-care, grooming, dressing, showering, bathing, toileting, feeding, and ambulation. + Housekeeping including tasks such as changing linens, laundry, and light cleaning. * Provides supervision and emotional support as needed for participants to perform necessary tasks. * Reports observed changes in participants’ conditions to the Home Care Supervisor and/or other PACE clinical staff. * Observes, tracks and reports participant status, including information such as vital signs, . nutritional intake. * Proactively communicates participant health and safety concerns to the Home Care Supervisor and/or other program staff. * Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions. * Interacts in friendly, courteous, and professional manner with participants and their families/other caregivers. * Participates in social and recreation activities with participants as appropriate. * Utilize Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic. * Performs other job duties as required by manager/supervisor. **Qualifications:** **Required:*** Must complete and pass the North East Medical Services PACE training curriculum upon hire. * Current BLS and First Aid required * Ability to communicate effectively. **Preferred:*** Minimum 1 years of experience as a Personal Care Aide or EMT, or certification as a Certified Nu
