Responsibilities may include: * Provide 1:1 skilled nursing care in the patient’s home * Complete ongoing patient assessments and monitor changes in condition * Administer medications as ordered * Monitor vital signs and patient response to care * Assist with G\-tube care, feeding support, and medication administration * Provide respiratory, tracheostomy, ventilator, ostomy, catheter, or wound care when applicable and within scope * Document care accurately and timely * Communicate with families, care teams, supervisors, and providers as needed *
