**Responsibilities**
* Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts when necessary.
* Monitor patients’ vital signs and observe for any changes in health status, reporting findings accurately to healthcare professionals.
* Prepare nutritious meals tailored to dietary needs and assist with feeding as required, ensuring safety and compliance with care plans.
* Medication reminders following prescribed schedules while adhering to HIPAA regulations and medication administration protocols.
* Support clients with cognitive impairments such as dementia or Alzheimer’s disease through memory care strategies and personalized engagement activities.
* Maintain detailed documentation of care provided, patient observations, and any incidents in accordance with home health standards.
* Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well\-being.
**Qualifications**
* Proven experience working in group homes, assisted living facilities, nursing homes, or home health settings supporting individuals with developmental disabilities or neurodevelopmental disorders.
