Mobile Home Health Aide

**Job description:** Millennium Home Health is looking for compassionate and reliable Certified Home Health Aides who want to build a rewarding career in mobile healthcare. Full time and part time schedules are available. The position is per diem and you will be paid between $30\-40 per visit, not per hour. You will be responsible for assisting in providing direct patient care under the supervision of a nurse or provider. Your responsibilities will include following the plan of care, assisting with treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. **RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:** * Provides direct patient care. * Assists in implementing the plan of care initiated by the nurse or provider. * Provides accurate and timely documentation consistent with the plan of care. * Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. * Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the nurse and/or Clinical Supervisor. * Uses equipment and supplies effectively and efficiently. * Participates in personal and professional growth and development. * Performs other duties as assigned by the nurse. * Maintains patient confidence and protects facility operations by keeping information confidential. * Maintains a safe and clean working environment by protecting patients and employees by adhering to infection\-control policies and protocols.
