Recognizes complex learning needs and prepares individual teaching plans for patients/families and consistently demonstrates effective patient education to peers. Consistently ensures a comprehensive continuum of care plan in collaboration with the health care team. Demonstrates and role models effective communication and conflict resolution skills.
