This full\-time opportunity offers professional growth, mentorship, and the ability to make a meaningful impact in the lives of patients throughout the community. **Responsibilities** -------------------- * Provide hands\-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols * Deliver patient care as ordered, including IV infusions, infusion pump management, and tube feedings under the supervision of a Registered Nurse * Implement and maintain patient and caregiver education plans under the direction of a Registered Nurse * Monitor patient conditions and communicate treatment responses and changes in status to the supervising Registered Nurse * Administer medications and evaluate patient responses when appropriate * Collaborate with interdisciplinary team members to ensure continuity and quality of care * Maintain accurate and timely documentation of patient assessments and care provided * Promote patient safety, independence, and positive health outcomes in the home environment **Qualifications** ------------------ * Graduate of an accredited Practical Nursing program * Current Licensed Practical Nurse (LPN) license in the state of practice * Current Basic Life Support (BLS) certification * Valid Florida driver's license or ability to obtain one within 30 days of hire * Associate Degree or Diploma in Nursing required * Minimum of one year of Licensed Practical Nurse experience required * Previous home health or home care experience preferred * Strong communication, organizational, and patient education skills * Ability to work independently and effectively manage patient visits * Must meet all ap
