Job description
**Role Summary**
The Caregiver II \-Certified Nursing Assistant (CNA) provides compassionate, hands\-on personal care and support to patients receiving home care and/or center\-based services. Under the direction and supervision of a Supervisor or Manager, this role supports patient dignity, safety, and independence while ensuring care is delivered according to individualized care plans.
In addition to direct care, the Caregiver II plays a leadership role by supporting onboarding and ongoing training of fellow caregivers. This position is ideal for an experienced caregiver who enjoys mentoring others and delivering high\-quality, patient\-centered care in a collaborative healthcare environment.
**Duties**
* Provide personal care and escort assistance in accordance with individualized care plans, including: Grooming, dressing, peri\-care, bathing, showering, and toileting, Housekeeping, laundry, meal preparation, feeding assistance, etc.
* Observe and report changes in patient condition to the team in a timely and professional manner.
* Demonstrate safe care practices by using proper body mechanics, adhering to universal precautions, and maintaining a safe environment.
* Interact professionally with patients, families, and team members, encouraging engagement in conversation and activities.
* Perform all duties of a Caregiver I as needed.
* Support onboarding and ongoing training of Caregiver I and Caregiver II staff using established training materials and manuals.
* Actively participate in departmental meetings and family meetings as required.
* Utilize safety equipment appropriately for lifting and transfers; provide positive guidance to others on proper use.
* Perform other related duties as assigned.
**Required Qualifications**
**Experience**
* Minimum of one (1\) year of experience working with elderly or frail populations.
* Experience working with seniors with dementia.
