WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community\-based services. The Personal Care Assistant is a vital part of our team, providing aid to our participants as well as contributing to the care planning process as a member of our interdisciplinary team (IDT).
