$110,000 – $140,000\+ per year DOE \| Full\-Time

We are seeking an exceptional **Nurse Care Supervisor** to help lead our caregiving team, oversee quality care delivery, support families, and ensure our clients receive the personalized, relationship\-centered care that sets CCC apart.
This is not a traditional nursing management position. It combines elements of:
* Clinical leadership
* Care management
* Caregiver coaching and development
* Family support and advocacy
* Quality assurance
* Client assessment and care planning
* Community\-based senior care
You will serve as a key leader within our organization, helping clients remain safe, independent, and engaged while supporting caregivers in delivering exceptional care.
**What You'll Do:**
* Conduct comprehensive in\-home client assessments
* Develop individualized care plans that promote safety, independence, wellness, and quality of life
* Monitor client progress and adjust care plans as needs evolve
* Evaluate home safety and provide recommendations to reduce risk and improve outcomes
* Support clients with complex medical, cognitive, emotional, and social needs
Caregiver Leadership \& Development
* Coach, mentor, and support our caregiving team, consisting primarily of Home Care Aides (HCAs) and Certified Nursing Assistants (CNAs)
