Job description

Case manager is responsible for providing, coordinating, monitoring, and evaluating treatment plans and their progress; facilitating interdisciplinary approaches; and monitoring progress for patients covered under worker’s compensation. Work closely with physicians, nurses, mental health and other health care professionals, as well as family of patients. Maintains client's electronic file. Makes appropriate referrals for patients to obtain needed services. Creates various reports. Extensive documentation is necessary to ensure that the individual receives needed services and makes progress towards goals. This position includes telephonic and field case management. This position includes report writing and summarizing documents. **QUALIFICATIONS** **Skills Required and Job Duties:** · Ability to analyze complex situations and recommend appropriate courses of action. * Strong verbal and written communication skills. Must be able to write cohesive and comprehensive documentation. Bilingual English and Spanish is preferred. * Ability to handle confidential and sensitive information in a professional manner with discretion and integrity. Understanding of HIPAA rules and compliance. * Ability to establish and maintain effective working relationships with clients, staff, community members and other stakeholders. * Ability to work in collaboration with others. * Ability to organize and conduct several tasks and projects simultaneously. * Schedule Flexibility. * Ability to handle and resolve conflicts. **Background Experience:** · Case Management experience required · **Registered nurse or** Bachelor’s degree from an accredited college or university with a major in a Nursing, Social, Behavioral Health or Human Service area.
