Job description

Ambulatory Care Clinical Coordinator / Pharmacist \- Hourly \- This position is responsible for coordinating effective use of the pharmacy benefit by delivering clinical and educational interventions designed to improve pharmaceutical care for patients assigned to Housecall Providers. Pharmaceutical care will focus on patients enrolled in primary care, palliative care, advanced illness care, and hospice care programs and will be primarily focused on caring for geriatric patients. This position will demonstrate consistent application of knowledge and skills to deliver appropriate, safe, efficient, and cost\-effective patient care as part of a multidisciplinary team. This pharmacist will play a key role in the patient\-centered medical home, incorporating principles of population\-based medicine and providing comprehensive medication management under established collaborative practice agreements. This position will coordinate the expansion of pharmaceutical care services within Housecall Providers primary care and palliative care services. Coordinating and leading the design, development, and implementation of quality improvement initiatives will be an important aspect of this position. NOTE: This is temporary position with an expected duration of 9 months.
