We’re seeking dedicated **Physical Therapists** (PTs) and **Physical Therapist Assistants** (PTAs) to join our compassionate home‑health team across the Bay Area and Sacramento regions. **Role Overview**
PTs and PTAs provide skilled therapy services to patients in their homes, supporting recovery, mobility, safety, and independence. You’ll collaborate closely with the interdisciplinary team to ensure coordinated, high‑quality care.
**Expanded Responsibilities**
**Physical Therapist (PT) Responsibilities**
* **Comprehensive Evaluations:** Conduct initial assessments, SOC/Admissions, and reassessments using standardized tests and clinical judgment.
* **Plan of Care Development:** Create, update, and manage individualized treatment plans aligned with physician orders and patient goals.
* **Skilled Interventions:** Deliver therapeutic exercises, gait training, balance programs, neuromuscular re‑education, transfer training, and pain‑management modalities.
* **Clinical Decision‑Making:** Modify treatment plans based on patient progress, safety concerns, or changes in condition.
* **Care Coordination:** Communicate with physicians, case managers, nurses, and other disciplines to ensure seamless care.
* **Patient \& Caregiver Education:** Teach home exercise programs, safety strategies, equipment use, and functional mobility techniques.
* **Documentation:** Complete accurate, timely visit notes, reassessments, and discharge summaries per agency and regulatory standards.
