Physical Therapist

**Overview** We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in physical therapy and a passion for improving the quality of life for patients across various demographics. This role involves working closely with patients to develop personalized treatment plans that promote rehabilitation and restore mobility, strength, and function. **Responsibilities** * Assess patients' needs and develop individualized treatment plans based on their specific goals and conditions. * Implement therapeutic interventions to improve patients' mobility, strength, balance, and range of motion, and to manage pain. * Collaborate with multidisciplinary teams to coordinate patient care and ensure comprehensive treatment approaches. * Educate patients and their families about therapeutic techniques, home exercise programs, and assistive devices. * Document patient progress and modify treatment plans as necessary to achieve optimal outcomes. * Participate in discharge planning to ensure continuity of care post\-treatment. * Maintain up\-to\-date knowledge of medical terminology, anatomy, and best practices in physical therapy. **Qualifications \& Experience** * Current state Physical Therapist license and a degree from an accredited physical therapy program (DPT or equivalent). * Proven experience in physical therapy, particularly in settings involving geriatrics and home health. * Familiarity with discharge planning processes and patient care protocols. * Previous experience in orthopedic or neurological rehabilitation is preferred. * Excellent communication skills with the ability to work effectively with patients, families, and healthcare professionals.
