Title: Social Worker\-VEPT Location: Upper East Side Org Unit: Research Work Days: Monday\-Friday Weekly Hours: 35\.00 Exemption Status: Exempt Salary Range: $91,200\.00 \- $99,800\.00 * As required under NYC Human Rights Law Int 1208\-2018 \- Salary range for this role when Hired for NYC Offices **Position Summary** -------------------- Serves as a social worker to the Vulnerable Elder Protection Team (VEPT), a first\-of\-its\-kind Emergency Department\-based consult service focused on optimizing care for potential victims of elder abuse. Supports program operations, assists with clinical management of cases, and participates in community outreach. Supports related elder abuse research and communications as needed. **Job Responsibilities** ------------------------ * Provide direct clinical care to elder abuse victims, including psychosocial assessments, supportive counseling to patients and concerned others, education, and referrals to community resources. * Coordinate triage and care between VEPT clinicians, interdisciplinary hospital teams, and community agencies to care for VEPT patients, including developing protocol for interactions with new hospital\-at\-home program. * Contribute to longitudinal follow up on medical, psychosocial and legal outcomes of VEPT patients via direct outreach and chart review, and provide relevant additional intervention. * Evaluate, refine, \& maintain VEPT clinical offerings based on frontline experience \& program evaluation analysis. * Maintain partnerships and integration with external agencies including APS, NYC Department for the Aging, and CEASe to strengthen elder protection systems. * Attends Multi\-Disciplinary Team (MDT) meetings in all NYC boroughs, serving as a representative of VEPT and presenting cases as appropriate. * Advance various research initiatives beyond VEPT that relate to serving vulnerable older adults, which may include co\-authoring book chapters and academic articles, presenting a
