Your active engagement and empathetic approach will make a meaningful difference every day, empowering clients to maintain independence and dignity. **Responsibilities** * Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts when necessary. * Monitor and observe patients’ conditions closely, documenting vital signs and changes in health status accurately within Electronic Health Record (EHR) systems. * Administer medications and perform catheter care following medical instructions while maintaining strict compliance with HIPAA regulations to protect patient privacy. * Support clients with meal preparation and nutritional needs, ensuring dietary guidelines are followed for conditions such as dementia or Alzheimer's disease. * Provide specialized care for individuals with developmental disabilities or those requiring memory or hospice care, including dementia and Alzheimer's\-specific techniques. * Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall health and safety. * Maintain detailed medical documentation and communicate effectively with healthcare teams regarding patient progress or concerns. **Requirements** * Proven experience in home health care, assisted living facilities, nursing homes, or long\-term care environments. * Strong knowledge of medical terminology, vital signs measurement, medication administration, and infection control protocols. * Maintaining medical assisted devices. * Certifications such as CPR (Cardiopulmonary
